Whether you or your loved one need minimal assistance, or a more thorough and comprehensive care plan, we are here to assist you each day with your personal activities of daily living. Medical assessments are done prior to admittance into our community and routinely thereafter to ensure we are providing the top-notch care you will come to expect. All care plans are determined with input by you or your representative, our staff, and your primary care physician; in concert, we believe we are able to provide the care necessary to achieve optimal success while maintaining dignity and respect. We have taken the time to think through the details, so residents can enjoy their lives as independently as they are able. At Sonnet Hill, the choice will always be yours… Our space is so vibrant and fresh that it’s easy to forget that we are fully licensed assisted living community. We aim to give our residents the ability to customize their lifestyle—including the type and level of help they receive. A full list of personal services is available, from laundering to assistance with dressing and medication management. Our on site caregivers work around-the-clock, and we can arrange for individual physical, speech or occupational therapy within our community. If your care requirements should change, so too will the care we provide. Sonnet Hill features a separated Memory Care portion within our community for residents that need extra care due to Alzheimer’s related dementia. All of our residents in this portion of the community are provided customized service plans provided by our trained care staff, available 24 hours a day with licensed nurse oversight. The service provided is available in a tranquil and compassionate environment, catered towards those with higher needs related to memory loss. Our focus here is on achieving the highest practical quality of life, while treating the residents in a manner that allows them to function as independently as possible, while receiving the personalized assistance and care they need. In our Memory Care apartments, Basic Personal Care is included in the rental rate. *Incontinence management is not included. Families are able to choose from both private and shared accommodations. For those loved ones and family members that need assistance with short-term respite stay accommodations, we provide that as well, with a fully furnished unit that includes all utilities, meals, and care services.
